背景技术:
在男人和女人中,内脏脂肪组织积累与非胰岛素依赖性糖尿病、心肌梗塞、中风和其它动脉硬化疾病和它们的相关风险因素发展的增加的风险相关,所述相关风险因素包括胰岛素抗性,升高的血脂,葡萄糖和高血压。这些风险因素的聚集已被表示为“代谢综合征”,也称为“综合征X”,“胰岛素抗性综合征”或“致命四部曲”。该综合征也被表征为一种或多种内分泌紊乱,因此也被称为“神经-内分泌综合征”(Marin,P.Neuroendocrine News,21(3)1996,2)。这些紊乱包括性激素(男人为睾丸激素,女人为雌激素)的低血清水平,生长激素的降低作用的征兆,和皮质醇的过渡分泌。如通过用皮质醇合成抑制剂酮康唑成功治疗所证明,后者在临床上已经显示作为代谢综合征发展的主要成因过程(WO 96/04912)。
与代谢综合征相关的病症包括II型糖尿病(IDDM),非胰岛素依赖性糖尿病(NIDDM),心肌梗塞,中风和其它动脉硬化疾病以及这些疾病的风险因素,通常是胰岛素抗性,由内脏脂肪组织积累导致的腹部肥胖,升高的血脂,和升高的心脏舒张和/或收缩血压。
已知内脏脂肪组织为腹部内脂肪,与向心性肥胖相关的脂肪储存。这种脂肪储存不同于位于全身的皮下脂肪储存。它是内脏脂肪组织,已经与疾病增加的风险以及死亡率相关。内脏脂肪组织通过以尚未确定的方式调节全身代谢在该过程中扮演重要的作用。在肥胖情形中,内脏脂肪组织的相对量可以随个体的差异而不同,并且精确确定内脏脂肪组织水平的唯一手段是通过利用磁共振成像和通过计算机的X线体层照相术。这些复杂技术可提供内脏脂肪组织水平的详细测定,但不适用于为了保健目的的全面测量社区的常规手段。另外,与MRI和CT扫描相关的费用是相当高的,因此不适用于常规排查。
如同所见,存在这样一种需求,即需要测量,检测和跟踪内脏脂肪组织水平的相对简单且成本高效的技术,作为诊断和可能治疗代谢综合征的方法以及作为发现治疗代谢综合征的潜在化合物的方法。
发明内容
依照本发明的一个方面,测量内脏脂肪组织水平的方法包含获得生物样品;和检测或测量多肽标记水平,所述多肽标记包含至少一种多肽,所述多肽选自由具有SEQ ID NO2,4,6,8,10,12,14,16,18,20,22,24,26,28,30,32,34,36和38的多肽组成的组。

本发明克服了鉴定适于作为用于疾病早期诊断的代谢综合征标记的基因和多肽以及对这类标记的长期需求的问题。令人惊讶地发现一组特异基因更多选择性地在内脏脂肪组织中分泌。表1中列出了差异表达的基因和它们编码的多肽,以及它们的登记号。

基于表1中列出的多肽,本发明提供了一种测量受试者体内存在的内脏脂肪组织的相对量的标记。然后可以将该测量与代谢综合征的诊断或代谢综合征的早期阶段相关联。这些标记包含至少一种多肽,其选自由表1中所列多肽(SEQ ID NOs2,4,6,8,10,12,14,16,18,20,22,24,26,28,30,32,34,36和38)组成的组。因此,本文所用术语“标记”是指一种或多种多肽,其在内脏脂肪组织中优势表达,并且能用于测量内脏脂肪组织的量,因此,可用于诊断代谢综合征,一种前代谢综合征状态或对发展为代谢综合征的敏感性。这些标记可单独使用或作为已知在内脏脂肪组织中表达的多个多肽的结合使用。
本文所用术语“多肽”是指氨基酸的聚合物,并不指特定的长度。因此,肽,寡肽和蛋白质均包括在多肽的定义内。
优选地,本发明的标记是包含至少一种多肽的标记,所述多肽选自由表1指列出的多肽组成的组。
关于在内脏脂肪组织中优势表达的多肽的定义,本发明提供了一种测量个体中内脏衍生的分泌蛋白质水平的体外方法。测量的蛋白质的量可外推至个体中内脏脂肪组织的总量,从而确定个体中内脏脂肪组织的水平。而且,通过测量内脏衍生的分泌蛋白质的水平,同样可以确定是否个体间内脏脂肪组织不同。而且,分泌蛋白质的差异可以与在不同个体中发现的不同发病率相关联。
按照本发明,术语“差异表达的”或“优势表达的”指表达主要由来源于内脏脂肪组织的组织和/或细胞分泌的蛋白质的标记基因。
按照本发明,如本文使用的术语“生物样品”表示包含其中可以测量标记基因的差异表达和可以获自个体的物质的样品。特别优选的样品包含体液如血液,血清,血浆,尿液,滑液,脊液,脑脊髓液,精液或淋巴液,以及身体组织,如内脏脂肪组织。
差异表达的标记基因的检测和/或测量可以包含特定核酸分子如RNA或cDNA的增加,减少和/或缺失的检测,表达的多肽/蛋白质的测量/检测以及表达的蛋白质/多肽的(生物)活性(或其缺失)的测量/检测。(生物)活性可包含酶活性,涉及信号途径-事件如抗原识别以及效应物-事件的活性。
在本领域公知检测/测量RNA和/或cDNA的方法,并且包含如后附实施例中所述的方法。这类方法包括,但不限于PCR-技术,RNA印迹,affymetrix芯片等。
如本文所用的术语“检测”指多肽缺失或存在的定量确定。如本文所用的术语“测量的”指在来自患者的生物样品中多肽表达差异的定量测定。另外,术语“测量的”也可指在来自内脏脂肪组织的生物样品指多肽表达差异的定量测定。
检测和/或测量生物样品指多肽的方法在本领域中是众所公知的,包括但不限于蛋白印迹,ELISA或RIA,或各种蛋白质组学技术。识别列于表1中多肽或其肽片段的单克隆或多克隆抗体可通过用纯化的蛋白质免疫兔来产生以检测多肽或肽片段,或可以使用已知识别多肽或肽片段的抗体。例如,能结合变性蛋白质的抗体,如多克隆抗体,可用于在蛋白印迹中检测本发明的肽。测定标记的方法的实例是ELISA。这种类型的蛋白质定量是基于能捕捉特异抗原的抗体和能检测该捕捉的抗原的第二抗体。另一种检测测量内脏脂肪组织水平的诊断标记的方法是通过分析活体解剖样本中存在或缺失本发明标记来进行的。检测这些标记的方法在本领域中是众所周知的,包括但不限于存在或缺失本发明标记多肽的免疫组织化学或免疫荧光检测。Harlow,E.和Lane,D.AntibodiesA Laboratory Manual,(1988),(冷泉港实验室出版社)描述了制备和使用抗体的方法和上文提及的试验。
尽管列于表1中多肽之一的分析可以精确诊断内脏脂肪组织的水平,但通过分析列于表1中多个多肽的组合可以提高诊断的精确度。因此,上文描述的体外方法包含一种标记,所述标记包含至少两种列于表1中的多肽。
为了诊断内脏脂肪组织水平,需要分析适当的生物样品存在或缺失标记。生物样品可以是血清,血浆,或各种包括脂肪组织的细胞的组织。来自脂肪组织的细胞可以通过任何已知方法来获得,如ERCP,促胰液素刺激,细针吸引术,细胞学灌注(cytologic brushings)和大孔穿剌活检。
也可通过检测和/或测量编码上文所述标记的核酸分子来诊断内脏脂肪组织水平。优选地,核酸分子是RNA或DNA。
在本发明的一个实施方案中,上文所述体外方法包含将至少一种编码已知具有升高的内脏脂肪组织水平的个体中多肽标记的核酸的表达水平与已知具有低或正常内脏脂肪组织水平的个体中相同核酸的表达水平进行比较。
在本发明的另一个实施方案中,上文所述体外方法包含将已知具有升高内脏脂肪组织水平的个体中标记的表达水平与已知具有低或正常内脏脂肪组织水平的个体中相同核酸分子的表达水平进行比较。在体外方法的更优选的实施方案中,标记的表达水平的增加预示发展代谢综合征的敏感性。
另外,在本发明的另一个实施方案中,本发明的体外方法包含一种方法,其中检测和/或测量步骤是通过检测和/或测量由列于表1中基因编码的蛋白质/多肽或其片段来进行。此外,这些检测/测量步骤包含本领域已知的方法,如尤其是蛋白质组学、免疫-化学方法如蛋白印迹,ELISA等。
优选地,在本发明的体外方法中,比较了至少两种列于表1中的标记基因的表达水平。
本发明还提供了鉴定和/或获得与列于表1中多肽相互作用的化合物的筛选方法,所述多肽在内脏脂肪组织中优势表达,所述方法包含在允许化合物与多肽相互作用的条件下将多肽与一种或多种化合物接触;和检测该一种化合物或多种化合物与多肽间相互作用的步骤。
对于与细胞表面上细胞膜相关的多肽,或作为跨膜或整合膜多肽表达的多肽,可以用不同方法检测化合物与多肽的相互作用,所述方法包括,但不限于利用正常表达多肽或其中过表达多肽的细胞的方法,如通过检测被待筛选的化合物标记的已知配体的替代。备选地,膜制剂也可用于检测化合物与这种多肽的相互作用。
在本文公开的方法中使用的相互作用分析可包含FRET-分析(如特别是在Ng,Science 283(1999),2085-2089或Ubarretxena-Belandia,Biochem.38(1999),7398-7405中描述的荧光共振能量转移),TR-FRET和如本文公开的生化分析。此外,可以使用商业化的分析如“Amplified LuminescentProximity Homogenous AssayTM”(BioSignal Packard)。其它的方法在本领域中是众所周知的,特别描述于Fernandez,Curr.Opin.Chem.Biol.2(1998),547-603。
“测试相互作用”也可通过本领域熟知的特异免疫和/或生化分析来进行,其包含,如下文所描述的均相和多相分析。利用读出系统的相互作用分析在本领域中是熟知的,包括特别是双杂交筛选(如尤其是在EP-0 963376,WO 98/25947,WO 00/02911;和在后附实施例中所举例说明中所描述),GST-下降柱(GST-pull-down columns),尤其如在Kasus-Jacobi,Oncogene 19(2000),2052-2059中所描述的来自细胞提取物的共沉淀试验,“相互作用-诱捕”系统(尤其如US 6,004,746所描述)表达克隆(如λgt11),噬菌体展示(尤其如US 5,541,109中所描述),体外结合试验等。其它的相互作用分析方法和相应的读出体系特别描述于US 5,525,490,WO99/51741,WO 00/17221,WO 00/14271或WO 00/05410。Vidal和Legrain(1999)在Nucleic Acids Research 27,919-929中描述、综述和概述了本领域已知的其它相互作用分析,其可以按照本发明来使用。
均相(相互作用)分析包含其中结合配偶体保留在溶液中的分析和包含分析如凝集分析。多相分析包含分析,尤其如免疫分析,例如酶联免疫吸附测定(ELISA),放射性免疫测定(RIA),免疫放射分析(IRMA),流动注射分析(Flow Injection Analysis)(FIA),流动活化细胞分选(FACS),化学发光免疫分析(CLIA)或电致化学发光(ECL)报告。
本发明还提供了一种鉴定和/或获得作为列于表1的多肽的激动剂或拮抗剂的化合物的筛选方法,所述多肽在内脏脂肪组织中优势表达,该方法包含步骤a)在允许化合物与多肽相互作用的条件下将多肽与通过上述筛选方法鉴定和/或获得的化合物接触;b)测定多肽的活性;c)测定如(a)中定义的在未与所述化合物接触的宿主中表达的多肽的活性;和d)将在b)和c)中所测定的活性定量相关联,其中与c)相比在b)中测定的降低的活性预示一种激动剂或拮抗剂。该筛选方法可作为体外分析或作为基于宿主的分析来进行。在本发明筛选方法中使用并且包含和/或表达列于表1中多肽的宿主可以包含原核以及真核细胞。这些细胞可以包含细菌细胞,酵母细胞,以及培养的(组织)细胞系,尤其是来源于哺乳动物。其它的动物也可作为宿主使用,如非人转基因动物。因此,可用包含核酸分子的载体转染和转化宿主(细胞),所述核酸分子编码如本文公开的内脏脂肪组织中差异调节的多肽。
因此,宿主细胞或宿主可因此用编码这种多肽的核酸分子或包含该核酸分子的载体遗传修饰。术语“遗传修饰”表示宿主细胞或宿主除了其天然基因组外包含编码列于表1中的多肽或至少其片段的核酸分子或载体。另外的遗传物质可被引入宿主(细胞)或引入其祖先/亲代之一中。核酸分子或载体可以作为基因组外的独立分子,优选作为能复制的分子而存在于遗传修饰的宿主细胞或宿主中,或它可以被稳定整合到宿主细胞或宿主的基因组中。
优选地,本发明还提供了一种用于鉴定和/或获得作为列于表1中多肽的拮抗物的化合物的筛选方法,所述多肽在内脏脂肪组织中优势表达。
如上文所提及,本发明的宿主细胞可以是任何原核或真核细胞。合适的原核细胞是通常用于克隆的那些,如大肠杆菌(E.coli)或枯草芽孢杆菌(Bacillus subtilis)。此外,在本文公开的筛选方法中也预示了这些原核宿主细胞。此外,真核细胞包括,例如真菌或动物细胞。合适的真菌细胞的实例是酵母细胞,优选那些酵母属和最优选那些酿酒酵母种类。合适的动物细胞是例如昆虫细胞,脊椎动物细胞,优选哺乳动物细胞,如CHO,HeLa,NIH3T3或MOLT-4。其它本领域已知的合适的细胞系获自细胞系保藏机构,如美国典型培养物保藏中心(ATCC)。
通过在存在化合物的条件下测定多肽的活性来鉴定该化合物,所述化合物与列于表1中的多肽相互作用,并且抑制或拮抗多肽。
如本文所用,术语“活性”涉及特定多肽的一种或多种功能性能。对于酶而言,术语“活性”涉及特定多肽的酶活性。对于粘附分子而言,术语“活性”涉及多肽的粘附性能,并且可以利用分析来确定,所述分析例如,但不限于粘附分析,细胞扩展分析或粘附分子与已知配体的体外相互作用。对于细胞骨架蛋白,术语“活性”涉及由这类多肽进行的细胞骨架蛋白的调节,或涉及它们结合到细胞骨架蛋白中。作为非限制性实例,凝溶胶蛋白调节肌动蛋白聚合的能力或细丝蛋白A促进肌动蛋白纤丝正交分支的能力可以利用体外肌动蛋白聚合试验来确定。关于调节细胞骨架蛋白结构的活性可通过作为非限制实例的细胞扩展试验,细胞迁移试验,细胞增殖试验或免疫荧光试验或通过用荧光标记的鬼笔毒环肽染色肌动蛋白纤丝来进一步确定。对于离子通道,术语“活性”涉及跨膜离子流(氯化物流(Chloride lux))。对于转录因子,术语“活性”涉及它们调节基因转录的活性。基因的转录活性克使用通常使用的试验如报道基因试验来确定。对于生长因子和激素或它们的受体,术语“活性”涉及它们各自结合它们的受体或配体和诱导受体活化和随后的信号级联放大的能力,和/或它涉及因子或受体介导最终由介导受体活化的生长因子或激素导致的一种或多种细胞功能的能力。可以通过通常已知的配体结合试验来确定结合于受体的生长因子或激素。可以通过检测受体自磷酸化或通过受体的下游信号介质的修饰或募集分析来确定受体活化(通过信号复合物的免疫沉淀和蛋白印迹)。由生长因子或激素和它们的受体调节的细胞功能可以是细胞增殖(如通过利用胸苷掺入或细胞计数来确定),细胞迁移试验(如通过利用改进的Boyden隔室来确定),细胞存活或编程性细胞死亡试验(如通过利用DAPI染色来确定),血管生成试验(如可商业获得的体外试验以测定内皮管形成)。除了这些试验,也可利用其它试验以确定这些和其它细胞功能。
如用本发明的方法鉴定和/或获得的抑制剂,拮抗剂,活化剂或激动剂特别用于涉及共同发病(comorbidities)的代谢综合征的疗法运用,预防和/或治疗。
列于表1中多肽的抑制剂或拮抗剂可以通过上述筛选方法来鉴定,当与在筛选方法中缺失化合物相比较,存在降低的活性时,预示为抑制剂或拮抗剂,所述降低的活性是在存在化合物的条件下确定的。
因此,用本发明的方法鉴定、筛选和/或获得的潜在抑制剂或拮抗剂包括分子,优选小分子,其结合、干扰和/或占据表达的标记基因上的相关位点,所述基因在内脏脂肪组织中优势存在。
进一步预计这类抑制剂干扰(功能性)上调标记基因或基因产物如反义构建体,核酶等的合成/生产。按照本发明的方法可以筛选和获得的抑制剂和/或拮抗剂包括,特别是,肽,蛋白质,包括DNA,RNA,RNAi,PNA的核酸,核酶,抗体,小有机化合物,小分子,配体等。
因此,差异表达的标记基因的抑制剂和/或拮抗剂可包含一种或多种抗体。该一种或多种抗体可包含单克隆抗体及多克隆抗体。而且,可以预计嵌合抗体,合成抗体及抗体片段(如Fab,F(ab)2,Fv,scFV),或化学修饰的抗体衍生物。预计抗体结合标记基因或它的基因产物和/或干扰它的活性。
另外,预计特异结合如本文定义的标记基因或干扰标记基因活性的寡核苷酸和/或适体(aptamers)为抑制剂和/或拮抗剂。如按照本发明所用的术语“寡核苷酸”包含编码和非编码序列,它包含DNA和RNA和/或还包含任何可行性衍生物。术语“寡核苷酸”还包含肽核酸(PNA),其含有具有酰胺主链键合的DNA类似物(Nielson,Science 274(1991),1497-1500)。可抑制和/或拮抗标记基因活性并且可用本发明的方法鉴定和/或获得的寡核苷酸可以是,特别地,使用本领域熟知并且可商购的合成仪容易化学合成,所述合成仪如ABl 394 DNA-RNA合成仪。另外,合成的~22nt的小干扰dsRNA(siRNAs)的可用于抑制基因表达。
补充于上文公开的筛选方法,本发明提供了鉴定和/或获得化合物的筛选方法,所述化合物为列于表1中多肽表达的抑制剂,所述多肽在内脏脂肪组织中优势表达,该方法包含步骤a)将表达多肽的宿主与化合物接触;b)确定多肽的表达水平和/或活性;c)确定多肽在如a)中所定义的宿主中的表达水平和/或活性,所述宿主尚未接触化合物;和d)将如在b)和c)中所测定的多肽的表达水平定量相关联,其中与c)相比,在b)中测定的降低的表达水平预示多肽表达的抑制剂。
列于表1中多肽表达的抑制剂是通过上文描述的筛选方法鉴定的,当与在筛选方法中缺少化合物的条件下相比,在存在化合物的条件下测定的蛋白质表达水平降低预示多肽表达的抑制剂。
如本文所用术语“表达”涉及列于表1中多肽的表达水平,所述多肽在内脏脂肪组织中优势表达。优选地,内脏脂肪组织细胞中表达水平高于例如皮下脂肪组织的至少2倍,更优选至少3倍,甚至更优选至少4倍,最优选至少5倍。

在本发明的范围内,针对列于表1中蛋白质的抗体或其抗原结合片段可用于测量内脏脂肪组织水平的体外方法。
为了有效进行诊断筛选,本发明提供了一种诊断患者内脏脂肪组织水平的试剂盒,其包含上述一种或多种抗体,或其抗原结合片段。由本发明提供的另一种试剂盒是诊断患者内脏脂肪组织水平的试剂盒,其包含上文所述一种或多种编码标记的核酸。由本发明提供的另一种试剂盒是用于筛选识别一种化合物,所述化合物激动或拮抗列于表1的任何多肽,或抑制该任何多肽的表达。
如上文所提及,该抑制剂和/或拮抗物还可包含小分子。然而,小分子也可由本文公开的方法鉴定为激活剂或激动剂。术语“小分子”涉及但不限于小肽,无机和/或有机物质或肽样分子,如包含D-氨基酸的肽类似物。

用本发明方法筛选的化合物不仅仅包含单一的分离的化合物。还预计用本发明的方法筛选化合物的混合物。还可能使用提取物,如,特别是,来自原核或真核细胞或生物体的胞内提取物。

另外,本发明提供了用本文公开的方法获得一种或多种化合物制备诊断患者内脏脂肪组织水平的诊断组合物的应用。例如预计在这类诊断组合物中使用如本文所公开的特异检测或识别差异表达的标记基因产物的特异性抗体,其片段或衍生物。另外,在诊断组合物中可使用特异的多个引物/引物对,其可检测和/或扩增本发明的标记基因。
因此,在药物及诊断组合物中使用的化合物可包含抗体,抗体衍生物,抗体片段,肽或核酸,如多个引物/引物对以及反义构建体,RNAi或核酶。因此,治疗有效量的至少一种针对至少一种蛋白质的抗体,或其抗原结合片段,或治疗有效量的至少一种选自由具有Seq ID Nos.2,4,6,8,10,12,14,16,18,20,22,24,26,28,30,32,34,36和38的蛋白质组成的组的蛋白质,蛋白质片段或肽可用于制备治疗代谢综合征的药物。
诊断组合物还可包含现有技术中已知的适于检测的制剂(means)。
本发明还提供了选自由SEQ ID No.1,3,5,7,9,11,13,15,17,19,21,23,25,27,29,31,33,35,和37的核酸组成的组的核酸作为诊断内脏脂肪组织积累的标记的应用;和选自由SEQ ID No.2,4,6,8,10,12,14,16,18,20,22,24,26,28,30,32,34,36,和38的蛋白质组成的组的多肽作为诊断内脏脂肪组织积累的标记的应用。

具体实施例方式
实施例使用Ultraspec_RNA(Biotecx,Houston,TX),依照制造商的方案提取总RNA,并用DNase处理使用RNeasy Mini试剂盒(Qiagen,Valencia,CA)来纯化。使用T7-T24引物,通过SuperScriptTM双链cDNA合成试剂盒(LifeTechnology,Rockville,MD)从10μg总RNA合成双链cDNA。使用锁相凝胶(Eppendorf,Westbury,NY)通过苯酚/氯仿/异戊醇提取纯化双链cDNA产物。使用体外转录(IVT)MEGAscriptTMT7试剂盒(Ambion,Austin,TX)将双链cDNA进一步转变cRNA,并用生物素酰核苷酸1标记。使用RNeasy Mini试剂盒(Qiagen,Valencia,CA)纯化体外转录产物,并如所述片段化(Wodicka L,Dong H,Mittmann M,Ho MH,Lockhart DJ.Genome-wideexpression monitoring in Saccharomyces cerevisiae.Nat Biotechnol1997;151359-67)。按制造商(Affymetrix,Santa Clara,CA)的建议进行片段化的体外转录产物与人基因组U95(HG-U95)Genechip_阵列组的杂交。
统计方法按Affymetrix’s MAS算法(Affymetrix Technical NoteNew StatisticalAlgorithms for Monitoring Gene Expression on GeneChip_ Probe Arrays.(2001))对报告的信号强度进行所有的数值分析。对实验中所有芯片每一个的总平均数进行标准化。不将基因进行单独标准化。

然后筛选模型计算和统计对照的结果以产生最终的目的基因。将有意义定义为整个模型的p-值少于0.001和特定对照的p-值少于0.01。选择p-值的截断值以便在仍然寻找大多数真正的阳性时控制假阳性(Sokal R.R.,Rohlf F.J.Biometry W.H.Freeman and Company.New York,New York.(1969))。
